Move, Add and Changes (MAC) Forms
Please find below a range of 2degrees MAC forms, used for requesting service as required.
All the PDF forms are editable and when completed should be sent to the email listed in the form.
Standard requests will be actioned within 3 business days.
Urgent requests will be actioned within 4 hours (subject to availability).

The following applies to Simple MAC requests:
- Can be carried out remotely by 2degrees without requiring attendance at a Customer’s Site
- Do not require a LAN or WAN outage
- A simple MAC is considered one (1) change to be carried out on one (1) device/service
The following applies to Complex MAC requests:
- A Complex MAC can be for multiple changes on multiple devices/services
- A quote to complete the work will be supplied within 48 business hours
- To accept a Complex MAC quote, you are requested to return a copy of the quote along with the completed form including a Purchase Order Number
- An Engineer will call or email to advise a timeframe for the work to be completed
